The future of care is Better at Home: Join the conversation

The future of care is Better at Home: Join the conversation: Better at Home banner

Keeping more people well at home and out of hospital is at the heart of Kent Community Health NHS Foundation Trust's (KCHFT) vision for the future of community healthcare.

Working alongside partners across Kent and Medway, we’re looking at how care closer to home, earlier support and stronger community services can help people remain independent and reduce avoidable pressure on hospitals.

KCHFT is opening up the conversation with a Better at Home webinar from 6 to 7pm on Thursday, 5 November giving people the opportunity to hear directly from healthcare experts, find out what these changes could mean and ask the questions that matter to them.

The webinar will give local people the opportunity to hear from experts involved in delivering and shaping care, find out what the Better at Home approach could mean for them and their families and put their own questions directly to the people at the heart of shaping the change.

It follows KCHFT’s recent Better at Home conference in Whitstable, which brought colleagues and partners from across Kent and Medway together to explore neighbourhood health, prevention and stronger partnership working.

Among the experts joining the webinar will be Dr Shelagh O’Riordan, President of the UK Hospital at Home Society and a consultant geriatrician at KCHFT. Shelagh is passionate about changing the way we support people living with frailty.

She said: “As people are living longer, there are more people living with frailty at home and in care homes. It is increasingly important we find ways to bring care closer to home, helping people stay independent and connected to the people and places that matter most to them.”

The webinar will explore what this changing approach to healthcare could look like in practice and how services can help people remain independent and receive care closer to home.

Whether people have questions about how healthcare is changing, want to understand what Better at Home could mean for them or a loved one, or are simply interested in the future of local health services in Kent and Medway, everyone is welcome to join.

Register for the Better at Home webinar on Thursday, 5 November.
